By checking the box below, I agree to all of the following:
I am either (a) the rider, and I am 18 years old or older, or (b) the parent or legal guardian of this rider, who is under 18 years old, and I give my consent on their behalf.
I give permission for the rider to take part in the Pedal Bike ride and the T-shirt printing sessions at Balance Point Studios the week of the ride. I understand the rider is responsible for one of the ride's bikes for the ride, including following the rider agreement above.
I understand that bike riding has risks of injury. I release Balance Point Studios, its staff and volunteers from liability for injuries that happen during normal participation.
Emergency medical treatment: I give permission to Balance Point Studios to seek emergency medical treatment for the rider if I, or the rider's emergency contact, cannot first be reached by phone. I will be responsible for any and all costs of medical attention and treatment. I will not hold Balance Point Studios, or any person associated with it, responsible for any medical claim arising from such treatment or medical attention.
Video & photo release: I give Balance Point Studios permission to take and use photos and video of the rider during the ride and related events in its promotional and commercial materials, including film, apparel and art, as well as print, its website and social media, without any further authorization or payment.
